Job summary

Mental Health Partnerships is hiring a Targeted Case Manager in Philadelphia. The role requires coordinating community-based services for individuals with persistent mental health challenges, and co-occurring conditions.

It includes helping secure resources, housing, benefits, and health services, with in-person and occasional home visits. The position is full-time with a 8:30 AM–5:00 PM schedule (Mon–Fri) and a hybrid option after 90 days.

Qualifications

• High School diploma with at least 12 social science credits; Bachelor preferred.
• Minimum 1 year of case management experience in a fee-for-service setting.
• Valid driver’s license and access to a car for daily work.
• Knowledge of local Mental Health System and ability to learn software tools.
• Ability to work independently, meet deadlines, and provide excellent customer service.

Responsibilities

• Collaborate with participants to set self-directed recovery goals.
• Assist with educational, employment, social goals and housing options per plan.
• Provide access to benefits, medical resources, and mental health services.
• Document progress in the electronic health records system and participate in community activities.
