Job summary

Mental Health Partnerships is seeking a Drug and Alcohol Specialist to serve as a Targeted Case Manager in Pottstown, PA. The role requires assessing participants with mental health and substance use challenges, developing plans, and linking them to community resources.

The position is full-time, hybrid, with in-person visits and one remote day per week, at a rate of $24/hour. The D&A Specialist works in collaboration with a care team, conducts weekly recovery groups, and documents progress

Qualifications

• Bachelor’s degree in a relevant field or RN with TCM duties limited to TCM responsibilities
• Minimum of 2 years in a behavioral health environment focusing on substance use
• Knowledge of local community and statewide substance use treatment resources
• Must have or obtain BHTEN certification within 1 year of hire
• Valid driver’s license and access to a car for daily work
• Able to qualify for and obtain a prison ID
• Basic computer skills including email, timesheets, and EHR systems
• Ability to work independently and meet deadlines while maintaining quality service
• Strong communication, cultural competence, and teamwork skills

Responsibilities

• Assess substance use and determine treatment needs and housing supports
• Develop individualized plans with the team to address treatment needs
• Link participants to substance use services and monitor engagement
• Provide crisis intervention and support to participants as needed
• Coordinate weekly recovery group activities
• Document progress in the electronic health records system
• Engage participants to enhance readiness for treatment and recovery
• Collaborate with participants to identify resources for community integration and self-sufficiency
• Create and review self-directed recovery and crisis plans with participants
