Job summary

South Middlesex Opportunity Council seeks a Case Manager to provide case management services for participants in a program serving episodically homeless individuals with relapse histories.

You will perform assessments, develop an Individual Service Plan within three days of entry, coordinate referrals, and maintain confidential client files while ensuring HUD requirements are met. Hybrid work is permissible (0-20% of the week) with a 9:00AM–5:00PM schedule.

Qualifications

• Bachelor’s Degree in related field. May be substituted with a minimum of 5 years working with the target population.
• Substance abuse experience required.
• Excellent interpersonal and organizational skills.
• Previous experience with group facilitation preferred.

Responsibilities

• Ensure adherence to policies and protocols for programs serving chronically homeless individuals.
• Develop regular program schedule including activities, meetings, meals, etc.
• Ensure HUD requirements are met and documentation is complete in client files.
• Conduct initial service needs assessment for new entrants.
• Develop Individual Service Plans within three days of entry.
• Collaborate with Shelter Staff, Relapse Prevention Managers and Sober Housing staff.
• Maintain daily contact with clients and regular 24-month outreach schedule.
• Maintain confidential client and agency information; comply with funder requirements.
