Job summary

Delta-T Group is seeking a Recovery Case Manager in the Kensington area of Philadelphia. The position is 85% hybrid or community-based, requiring a valid driver’s license and access to a vehicle. Candidates must have lived experience with homelessness and substance dependency, and be prepared to work with participants facing D&A challenges.

Qualifications

• Bachelor's degree in Human Services or related field.
• 1+ year experience as case manager.
• Lived experience with homelessness and dependency (drug & alcohol abuse).
• Ability to engage with adults dealing with addictions.

Responsibilities

• Collaborate with participants to formulate and review a comprehensive service plan.
• Provide outreach, service coordination, counseling, advocacy, and other needed services in various environments (streets, shelters, prisons, hospitals, homes, offices).
• Deliver consumer-driven services such as housing placement, independent living skills, vocational guidance, and appointment escorts.
• Conduct regular home visits and accompany clients to medical appointments and treatment facilities as appropriate.
• Perform other duties as assigned.

One of our clients is seeking Recovery Case Manager in the Kensington area of Philadelphia.

Position is 85% hybrid or out in the community. Client provides vehicles to travel so valid driver's license is mandatory. Must have lived experience with homelessness & substance dependency. Participants are still dealing with D&A dependency, so this can be a very trying opportunity if not experienced in this area or if not prepared to deal with this population.

9:00 am- 5:00 pm Monday - Friday

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

• Collaborate with participants in the formulation of a comprehensive service plan that is reviewed and modified with that person on a regular basis.
• Provide outreach, service coordination, counseling, advocacy, and other needed services to participants in any environment including the streets, shelters, prisons, hospitals, apartments, office, etc.
• Provide individually tailored consumer driven services to each client...i.e. housing placement, independent living skills, vocational guidance, appointment escorts, family counseling, etc.
• Regular home visits, as well as accompany clients to medical appointments and treatment facilities as appropriate.
• Other duties as assigned.

CLIENT'S REQUIRED SKILLS & EXPERIENCE

• Bachelor Degree under the Human Services umbrella (criminal justice, psychology, social work, sociology, counseling)
• 1+ year experience as case manager
• Lived experience with homelessness and dependency(drug & alcohol abuse)
• Ability to engage with adults dealing with addictions
• Valid driver's license & vehicle
