Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator to engage and enroll members, assess needs, and connect them with ISC services. You will advocate for housing, health benefits, and community resources, performing outreach and coordinating care in a psychosocial rehab framework.

The role requires a HS diploma or GED, at least one year of relevant experience, and a willingness to drive.

Qualifications

• Bilingual Spanish speaking ability highly preferred.
• High school diploma or GED required.
• Minimum of one year of experience providing psychosocial rehab related services to a disabled population required.
• Bachelor degree in a human related field preferred.
• Driving with current class C driver's license, a vehicle for transportation, proof of valid automobile insurance are conditions of employment.
• Must possess a high level of tolerance, patience, and understanding for individuals with multiple needs.
• Must function well in a team-oriented approach to member services.
• Knowledgeable of harm reduction philosophy and motivational interviewing.
• Ability to develop, nurture and maintain good community relations on behalf of the organization.
• TB clearance and annual re-testing are required; CV vaccination policy subject to change.

Responsabilités

• Engage and enroll new members. Develop a Recovery Plan with each member that identifies personal needs and goals.
• Mediate and advocate to help members obtain housing, health care benefits, entitlements, etc.
• Visit members at residence, hospital or jail to help them live successfully and prevent homelessness.
• Transport members to appointments and errands when appropriate.
• Provide coaching, counseling, mentoring, instruction and feedback in living skills.
• Devise and implement outreach strategies for members who tend to isolate themselves.
• Provide timely information and feedback to program manager on issues.
• Assist members and team in achieving community integration, education and employment goals.
• Participate in staff and community meetings and day-to-day program operations.
• Complete program documentation and paperwork with required turnaround times.
