Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator/Intensive Case Manager to provide high-quality ICMS to clients eligible for permanent housing under the HFH program. The role emphasizes client-centered service, strong documentation, and collaboration with DHS and housing partners.

The ideal candidate will have a high school diploma or GED with two years’ related experience, be proficient in Word/Excel/Outlook, and ideally be bilingual in Spanish.

Qualifications

• High school diploma or GED required; 2 years’ psychosocial rehab-related experience preferred.
• Bilingual Spanish preferred.
• Proficiency with Microsoft Word, Excel, and Outlook.

Responsibilities

• Provide Intensive Case Management Services for a caseload of clients in housing programs.
• Assist clients with moving from homelessness to permanent housing and connect to needed services.
• Maintain timely documentation in EHR and ensure compliance with program standards.
• Coordinate referrals with DHS, HFH partners, and housing subsidies, and support access to medical and behavioral health services.
