Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator to support members through intensive case management, outreach and engagement, and access to services. The role emphasizes psychosocial rehabilitation and housing stabilization in the ISC program.

Under supervision, you will assess needs, develop Recovery Plans, coordinate with housing, health care benefits, and community supports, and help members transition to appropriate levels of care while promoting

Qualifikationen

• High school diploma or GED required.
• Minimum one year experience providing psychosocial rehab related services to a disabled population.
• Bachelor degree in a human related field preferred.
• Must demonstrate tolerance, patience, and understanding in busy work environments.
• Ability to develop and maintain strong community relations.
• Knowledgeable in harm reduction and motivational interviewing.
• Team-oriented approach to member services.
• Proficient with MS Word, Excel and Outlook.
• Flexible schedule including some evenings/weekends; on-call when needed.
• 3+ years driving history with a valid license and vehicle; insurance coverage required.
• TB clearance and annual re-testing required.
• COVID-19 vaccine not currently required.

Aufgaben

• Engage and enroll new members; develop a Recovery Plan with each member.
• Mediate and advocate to help members obtain housing, benefits, and entitlements.
• Visit members at home, hospital, or jail as needed to support stability.
• Transport members to appointments and errands when appropriate.
• Provide coaching, counseling, mentoring on living skills and money management.
• Devise outreach strategies for members who tend to isolate themselves.
• Provide timely information and feedback to program manager on issues.
• Assist members and team in achieving community integration and employment goals.
• Participate in staff and community meetings and daily program operations.
• Complete program documentation with required turnaround.
