Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator to join our outreach-focused team in California. The role focuses on street outreach, case management, and helping homeless individuals access housing and services.

The coordinator will develop recovery plans, coordinate transportation, and support daily living skills while working within a multidisciplinary team in a fast-paced environment.

Qualifications

• High School Diploma or GED required; Bachelor's preferred.
• Bi-lingual applicants preferred.
• Proficiency with MS Word, Excel and Outlook.
• Ability to provide outreach psychosocial rehab services to homeless/disabled populations.

Responsibilities

• Work collaboratively within an integrated mobile team to provide street outreach and linkage to housing.
• Provide outreach and engagement to homeless individuals through intensive, sustained processes.
• Develop and complete mental health treatment recovery plans with member goals.
• Provide case management to help secure independent stable housing and address barriers.
• Provide transportation to services and accompany members on public transit when needed.
• Deliver life skills training to support daily independent living.
• Participate in trainings and QA processes to improve service delivery.
• Advocate on behalf of members for housing, healthcare benefits and entitlements.
