Job summary

Mental Health America of Los Angeles (MHALA) seeks a Personal Service Coordinator to assess and engage potential members for ISC services, helping them access needed supports through intensive case management. You will visit members, arrange housing and benefits, transport to appointments, and coach on life skills, aiming to foster independence and community integration.

Bilingual Spanish preferred; HS diploma required; 1 year of relevant experience; driving required; flexible schedule; CPR

Qualifications

• Requires high school diploma or GED.
• Minimum of one year of psychosocial rehab related experience.
• Spanish bilingual ability highly preferred.
• Valid driver’s license and access to a vehicle.
• Knowledge of Microsoft Word, Excel and Outlook.
• Flexible work schedule including some evenings/weekends.
• TB clearance annually.
• CPR certification preferred.
• Ability to work in a team-oriented environment.

Responsibilities

• Engage and enroll new members. Develop a Recovery Plan with each member.
• Mediate and advocate to help members obtain housing, health care benefits, entitlements, etc.
• Visit members at home, hospital or jail as needed.
• Transport members to appointments and errands as appropriate.
• Provide coaching on money management, grooming, hygiene, meals, transport, and living skills.
• Devise and implement outreach to isolated members.
• Provide timely feedback to program manager on issues.
• Assist members and team in community integration, education and employment goals.
• Participate in staff and community meetings and day-to-day operations.
• Document program activities with required turnaround and records.
