Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator/Intensive Case Manager to provide high-quality ICMS to clients eligible for permanent housing under the HFH program. You will support clients with housing stability, healthcare access, and benefits navigation in a client-centered approach.

The role requires a high school diploma or GED with 2 years of rehabilitative experience; bilingual Spanish is preferred.

Qualifications

• High school diploma or GED required, 2 years' psychosocial rehab-related experience preferred.
• Bilingual Spanish speaking preferred.
• Proficient in Microsoft Word, Excel, and Outlook.
• Experience with HMIS and Electronic Health Records (EHR) systems.

Responsibilities

• Provide Intensive Case Management Services to a caseload of clients entering permanent housing.
• Assist clients with move-in, documentation, and enrollment into ICMS.
• Coordinate housing stabilization, access to health and behavioral health services, and benefits.
• Document case notes in Electronic Health Records per program standards.
• Maintain flexibility to work some evenings or weekends as needed.
