Job summary

Mental Health America of Los Angeles (MHALA) seeks a dedicated Personal Service Coordinator/Intensive Case Manager to provide high-quality ICMS to clients pursuing permanent housing through HFH. You will support move-in, income assistance, and access to health and behavioral health services in a client-centered framework.

The role emphasizes collaboration with DHS/HFH programs, documentation in EHR, and flexible scheduling, including nights or weekends as needed.

Qualifications

• Provide high-quality Intensive Case Management Services (ICMS) to clients.
• Assist with housing stabilization and documentation for subsidies.
• Ability to work in a team-oriented setting and provide client-centered service.

Responsibilities

• Provide caseload management for 40 clients in stable housing.
• Coordinate move-in and tenancy orientation with housing partners.
• Link clients to health, behavioral health, and social services.
• Document assessments and plans in EHR per DHS/HFH standards.
• Participate in on-call crisis prevention rotation and professional development.
