Qualifications

• Bachelor's degree in psychology, Social Work, Child Development, Human Services, Sociology, Counseling, or a related field preferred.
• Valid California Driver's License, proof of automobile insurance, good driving record.

Responsibilities

• Maintains an assigned caseload and provides individualized case management based on client strengths, needs, acuity, goals, and level of care.
• Participates in outreach, engagement, re-engagement, and warm handoffs, following referral and enrollment timelines.
• Collaborates with clients, families, and the treatment team to identify behavioral health, housing, benefits, education, employment, and other needs.
• Provides rehabilitation and skill-building interventions to strengthen daily living, communication, self-advocacy, and community functioning.
• Assists with housing, benefits, transportation, identification documents, and community-resource linkage.
• Uses trauma-informed, recovery-oriented approaches to support co-occurring mental health and substance use needs.
• Provides crisis prevention and safety planning, with escalation when needed.
• Offers field-based services in approved settings based on client preference and program requirements.
• Participates in treatment planning, supervision, and care transitions; coordinates with multidisciplinary teams.
• Completes accurate documentation and meets productivity, data reporting, and quality assurance requirements.
• Mantains confidentiality and professional boundaries, following HIPAA and agency requirements.
• Performs other duties as assigned.
