Job summary

ERGII, LLC seeks a Case Manager in a residential treatment facility in California. The case manager supports clients transitioning through treatment, develops individualized care plans, coordinates services, and documents progress in the EMR.

Responsibilities include assessments, discharge planning, crisis coordination, group sessions, and referrals to dental, psychiatry, therapies; requires CAADAC/RADT or licensure, CPR, TB clearance, and a clean driving record.

Qualifications

• CAADAC or RADT certification required in California.
• LIC or LMFT-Associate licensure in CA preferred.
• Minimum 1 year as a Case Manager with strong references.
• Current First Aid and CPR certification.
• Valid driver's license and clean record.
• TB test clearance and state health screening.
• Criminal background clearance as part of hiring.
• Knowledge of CA regulations and ERG guidelines.

Responsibilities

• Assess clients’ mental and physical health and plan care with client/family input.
• Coordinate and implement the treatment plan; document progress in EMR.
• Discharge planning and coordination with community resources.
• Facilitate group sessions and crisis intervention as needed.
• Prepare disability forms and coordinate external referrals.
