Job summary

Three Rivers Youth in Pittsburgh seeks an experienced Case Manager to support individuals with substance use disorders. You will work with clients to develop their recovery plan within 24 hours, help them meet goals, and track progress weekly using ROSC principles.

The role emphasizes collaboration with community resources, maintaining confidentiality, and ongoing reporting. A Master’s degree is preferred, CADC or related experience valued; valid PA driver’s license required.

Qualifications

• Master's degree in a related field preferred.
• Bachelor's degree in social work, psychology, criminal justice, social science, or related field required.
• CADC certification or relevant substance use disorder experience valued.

Responsibilities

• Collaborate with clients to develop recovery plans within 24 hours and monitor progress weekly.
• Coordinate with community organizations for resources, referrals, and services; maintain updated resource guides and housing lists.
• Provide screening and intake to determine need for higher level of care or evaluation.
• Follow up with clients and referral sources to ensure smooth transitions to intervention/treatment.
• Utilize motivational interviewing and ROSC in retention and engagement strategies.
• Organize social and cultural activities to promote sobriety and awareness of substance use issues.
• Maintain confidential participant records and prepare monthly, quarterly, and annual reports.
