Cumberland Heights is seeking a Recovery Care Advocate to support patients from admission through the first year of recovery. The role focuses on building trust, coordinating with the care team, and ensuring a smooth transition after discharge.

The position emphasizes empathy, strong communication, and the ability to work with patients and families to maintain progress and safety across the continuum of care.

Qualifications

• High school diploma or GED required.
• CPRS certification preferred but not required.

Responsibilities

• Meets face-to-face with patients for initial orientation and program features.
• Builds patient trust through empathetic interactions and recovery principles.
• Collaborates with treatment team and family to support goals and discharge plans.
• Conducts post-discharge follow-up with patients using phone, telehealth, and in-person contacts.
• Documents patient progress and ensures confidentiality.
