Job summary

Cumberland Heights Foundation seeks a Recovery Care Advocate to support the interdisciplinary plan of care from admission through the first year of recovery. You will build trusting relationships and provide recovery-oriented guidance to patients and families.

The role emphasizes empathy, crisis de-escalation, documentation, and collaboration with counselors, case managers, and other staff. A high school diploma and recovery experience are required; CPRS is preferred.

Qualifications

• HS diploma or GED required
• Minimum 2 years of recovery experience with active 12-step principles
• CPRS certification preferred but not required
• Valid Tennessee driver’s license preferred
• Proficient in Microsoft Office
• Ability to problem-solve, analyze, and interpret information
• Excellent organizational skills and ability to prioritize multiple tasks
• Ability to adapt to changing circumstances in a fast-paced environment
• Knowledge of cultural/age/population characteristics in treatment settings
• Strong written and oral communication in English

Responsibilities

• Meets face-to-face with patients for initial orientation to Recovery Support Services</br>Provide empathetic interactions and clinically accepted care
• Consults with patients on living environment to identify recovery capital
• Collaborates with program coordinator, counselor, case manager and staff on goals and discharge plans
• Conducts informational presentations about Recovery Support Services to groups as needed
• Develops mutual follow-up agreements for post-discharge contacts
• Performs post-discharge contacts via phone, telehealth, or in-person visits
• Communicates progress with family, providers, and referred sources within privacy rules
• Documents all interactions and recovery-related information accurately
• Promotes a safe, healing environment and supports quality of services
• Recommends improvements to service delivery and handles funds appropriately
• Ability to deescalate crises and assist in safety planning
