Job summary

Cumberland Heights Foundation is seeking a Recovery Care Advocate to support the interdisciplinary plan of care from admission through the first year of recovery post-discharge. The role emphasizes building trust with patients, coordinating with counselors and family, and delivering recovery-oriented services in a safe, healing environment on the Nashville campus.

Qualifications

• High school diploma or GED required.
• Minimum two years of recovery experience with 12-step principles.
• CPRS certification preferred but not required.
• Excellent written and oral communication in English.

Responsibilities

• Meet patients for initial orientation about Recovery Support Services.
• Develop trust through empathy and recovery principles.
• Collaborate with treatment team and family to support discharge plans.
• Conduct post-discharge follow-ups and maintain documented records.
• Provide crisis support and ensure confidentiality.
