Job summary

Pathways Vermont is seeking a Re-entry Services Coordinator to support individuals transitioning from incarceration to the community. You will provide life skills coaching, housing navigation, and access to benefits to promote community inclusion and permanent housing.

The role is full-time and in-person across Windham and Windsor Counties. A $2,000 hiring bonus is available after the introductory period, with emphasis on documentation and client-centered service.

Qualifications

• Ability to work within a client centered, strengths-based framework and advocate for clients.
• Excellent communication, writing, and computer skills required.
• Valid driver’s license and reliable vehicle for work.
• Continued employment depends on PREA certification and background checks.
• Lived experience with homelessness or mental health challenges is an asset.

Responsibilities

• Engage in relationship building/rapport building with clients using a client-centered approach.
• Provide one-to-one support and counseling on life skills, tenancy, budgeting and housing readiness.
• Assist clients in navigating housing paths including search, applications and lease understanding.
• Educate and advocate regarding benefits and entitlements (Social Security, Food Stamps, etc.).
• Maintain records, compile reports and complete program documentation.
• Collect and maintain housing history data relevant to housing stability.
• Support lease-ups, move-ins/outs and inspections with housing authorities.
• Attend annual staff and program retreats; retreats are fully paid.
• Occasional travel to other counties to support participants and partners.
• Perform other duties as assigned.
