Job summary

Pathways Vermont seeks a Re-entry Services Coordinator to support individuals transitioning from incarceration. The role focuses on life skills, housing access, and resources to promote community inclusion and permanent housing.

The position is in-person across Windham and Windsor Counties with a 35-40 hour workweek and a $22-$24 hourly wage, plus a $2000 hiring bonus. Qualified candidates bring strong communication, documentation, and advocacy skills, a valid driver’s license, and a commitment

Qualifications

• Ability to work within a client-centered, strengths-based framework.
• Excellent communication, writing, and computer skills.
• Ability to structure time, develop priorities, and document interactions.
• Valid driver’s license and reliable vehicle for work purposes.
• Ability to lift and carry up to 50 pounds; PREA certification and background checks may apply.
• Lived experience of homelessness or mental health challenges is an asset.

Responsibilities

• Engage in relationship-building with clients using a client-centered, strengths-based approach.
• Provide one-to-one support and counseling on life skills, tenancy, budgeting, and housekeeping.
• Assist clients in navigating housing paths, including searches, applications, and lease understanding.
• Educate and advocate regarding benefits and entitlements (SSDI, food assistance, Hireability, ICAN, etc.).
• Maintain records and complete program documentation, reports, and case notes.
• Support housing moves, inspections, and landlord interactions.
• Attend all-staff and programmatic retreats; travel to counties as needed.
• Perform other duties as assigned.
