Job summary

Enhabit Home Health & Hospice seeks a Clinical Documentation Reviewer to review and approve patient information submitted by Licensed Practitioners during start of care and recertifications. You will partner with branch leadership for ongoing staff education to ensure regulatory compliance and high-quality documentation.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Must hold a valid clinical license per state requirements.
• Minimum of one year of experience.
• Experience in Medicare home health or hospice setting preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including Microsoft Office.
• Experience with Homecare Homebase (HCHB) preferred.
• Dependable transportation and ability to drive in all weather conditions.
• Oral and written communication, fluency in English, active listening.
• Intermediate technology skills; Word and Excel proficiency.

Responsibilities

• Review and approve all patient information submitted by the Licensed Professional (LP) during start of care, re-certification, resumption of care, or evaluation visit.
• Provide ongoing education and training to branch LPs to ensure documentation compliance with regulatory standards.
• Conduct one-on-one training sessions when necessary.
• Promote quality patient care and maintain strong relationships with referral sources, patients, caregivers and staff.
