Enhabit Home Health & Hospice is hiring to support patient care across the home health and hospice continuum. The role focuses on reviewing and approving patient information at key care milestones, and collaborating with branch leadership to educate staff on documentation to meet regulatory standards.

Ideal candidates will be graduates of an approved nursing or physical therapy program, hold a valid clinical license, and have at least one year of Medicare home health or hospice experience, plus

Qualificações

• Must be a graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements.
• Minimum of one year of experience in Medicare home health or hospice settings.

Responsabilidades

• Review and approve all patient information submitted by the Licensed Professional during start of care, recertification, resumption of care or evaluation visit.
• Partner with branch leadership to educate and train LPs on documentation requirements to meet regulatory standards.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
