Job summary

Enhabit Home Health & Hospice is seeking a licensed professional reviewer to verify patient information during start of care, re-certification, resumption of care, or evaluation visits. You will partner with branch leadership to educate LPs and ensure documentation meets regulatory standards.

This role emphasizes collaboration with referral sources and ongoing staff training. Applicants should have a valid nursing license, CPR certification, and at least one year of Medicare home health or

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements to practice.
• Minimum of one year of experience in Medicare home health or hospice setting.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Basic demonstrated technology skills including mobile devices.
• Preferred experience with packet review/quality improvement and with coding.

Responsibilities

• Review and approve all patient information submitted during start of care, re-certification, resumption of care, or evaluation visit.
• Partner with branch leadership to provide ongoing education and training to LPs to meet regulatory standards.
• Conduct one-on-one training with LPs when necessary to ensure documentation accuracy.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and staff.
