Responsibilities

• Conduct clinical record reviews and audits for home health and hospice patients.
• Assess documentation for completeness, accuracy, and regulatory compliance.
• Identify gaps, risks, and opportunities for improvement.
• Provide feedback to clinicians and leaders to drive improvements.
• Support development and monitoring of Performance Improvement projects.
