Job summary

Hills & Dales in Dubuque, IA is seeking a Qualified Intellectual Disabilities Professional (QIDP) to coordinate, integrate, and monitor the active treatment program for each resident in line with ICF/ID regulations and CARF standards.

The QIDP will direct work and provide supervision and leadership of Personal Assistants through mentoring and development of a cohesive team, and will facilitate IPP meetings, quarterly reviews, and monthly progress notes.

Qualifications

• Bachelor's degree in social work, human services, or related field as defined in ICF/ID regulations.

Responsibilities

• Coordinate, integrate, and monitor the active treatment program for each resident in line with ICF/ID regulations and CARF standards.
• Direct the work of Personal Assistants, provide supervision and leadership, mentoring, and development of a cohesive team.
• Facilitate IPP meetings, quarterly reviews, monthly progress notes, and core team meetings for each resident.
• Prepare and maintain records and reports, ensuring compliance with policies and regulations.
• Attend trainings, communicate with families, and participate in organizational meetings and projects.
