Job summary

New Hope is seeking a QIDP to join its leadership team at the New Hope Intermediate Care Facility (ICF/ID) in Carroll, IA. The QIDP will coordinate service design and support for about 18 adults with intellectual disabilities, acting as the person’s advocate and liaison with families and funding agencies.

The role requires a 4-year degree in a human services field or RN, strong organizational and communication abilities, and at least one year of related experience.

Qualifications

• Bachelor's degree in human services or RN with at least one year of experience working with people with disabilities.
• Strong organizational and communication skills; self-directed.

Responsibilities

• Coordinate service design and support for individuals with intellectual disabilities.
• Advocate for each person and liaise with families, facility, and external funding agencies.
• Coordinate each person’s total plan within assigned caseloads.
