Position Summary
The Qualified Intellectual Disabilities Professional (QIDP) is responsible for coordinating, integrating, and monitoring the active treatment program of each assigned resident in a manner consistent with current federal, state, and local ICF/ID regulations and CARF standards. The QIDP will direct the work and provide supervision and leadership of assigned staff through mentoring and development of a cohesive team.
Key Duties and Responsibilities
- Provide ICF/ID Facility orientation to new Personal Assistants.
- Facilitate Individual Program Planning (IPP) meetings, quarterly reviews, monthly progress notes, and core team meetings for each assigned resident.
- Review resident progress reports from other homes as assigned.
- Prepare and facilitate resident admissions and discharges.
- Participate in referral and pre-admission visits with potential residents and families.
- Arrange for new admissions by completing new-admission checklists and providing staff training prior to admission date.
- Monitor and ensure performance indicators comply with Hills & Dales policies, federal, state, local, managed care organization (MCO)/Medicaid rules and regulations, and meet organizational quality standards.
- Ensure Personal Assistants follow proper safety and infection control procedures during assigned shifts.
- Constantly assess staff performance on quality care issues.
- Review and attend to Personal Assistants' daily concerns; provide continuous direction and supervision.
- Direct the work of Personal Assistants, providing necessary training and feedback on performance.
- Address complaints, resolve problems, and communicate issues to supervisors and upper management as appropriate.
- Maintain harmonious relations among staff.
- Review and approve Personal Assistants' incident report data recording.
- Prepare and maintain appropriate records and reports.
- Maintain knowledge of applicable state and federal regulations governing ICF/ID facilities.
- Assist department supervisors, managers, and directors as needed and perform other duties as required.
- Follow and maintain established department/center policies, procedures, quality, safety, environmental, and infection control standards.
- Attend training, workshops, and seminars.
- Communicate with families as appropriate.
- Participate in organizational meetings, committees, and special projects as assigned.
- In the absence of the Facility & Support Director, respond to all resident, staff, and property emergencies.
- Document circumstances of resident and staff incidents and initiate necessary intervention and follow-up.
- Schedule staff to provide and deliver necessary services and interventions, including reviewing DAS for upcoming open shifts.
- Provide documentation of services via approved computer software or handwritten records to record significant events, medication records, and other documentation according to individual programs, policies, and procedures.
- Support LTC Program staffing needs and work closely with LTC/DH Coordinator.
- Schedule staff to attend medical appointments and community outings.
- Maintain knowledge of individuals' basic and behavioral needs; use this knowledge to educate staff and deliver services consistent with individual program plans.
- Assure confidentiality of all data, including resident, employee, and operations data.
Requirements
- Free from convictions or prior employment history of child or resident abuse or dependent adult abuse.
- Bachelor's degree (B.A. or B.S.) from a four-year college or university in social work, human services, or related field as defined in the ICF/ID regulations.
- Minimum of one year experience working directly with persons with intellectual and/or developmental disabilities.
- Valid driver's license and ability to drive agency vehicle(s).
- Offer of employment contingent upon successful completion of criminal background checks and drug screening.

Pay Range:$58,411.40 - $61,006.40 per year
