Job summary

Provident Place in Apple Valley, CA seeks a Qualified Intellectual Disability Professional (QIDP) to lead the development and implementation of individualized care plans for residents with developmental disabilities. You will oversee operations of two licensed Intermediate Care Facilities and provide guidance to Program Managers to ensure high-quality, compliant care.

Qualifications

• Bachelor’s degree in Human Services or Social Sciences.
• Experience with individuals with developmental disabilities in residential or home & community care settings.
• Knowledge of CDPH rules and Regulations (Title 17 and Title 22).
• CPR certification.
• Pass criminal background test, pass physical and TB test.
• Strong management skills with supervisory experience in healthcare or residential care facilities.
• Proficiency in Microsoft Office for documentation and reporting.

Responsibilities

• Ensure all data collected and all Policies and Procedures followed.
• Manage all ISP schedules and HRC meetings.
• Monthly In-Services and implementation at each home.
• Ensure PMs are meeting the requirements of their job.
• Coordinate consultant assessments and recommendations for ISP planning.
• Schedule and conduct IDT meetings.
• Create/modify schedules for Direct Care Staff and ensure coverage.
• Work as Direct Care Staff as needed including weekends, nights and holidays.
• Ensure PMs avoid overtime and complete client data daily.
• Distribute ISP reports to relevant stakeholders.
• Coordinate services with Regional Centers and Day Programs.
• Manage hiring—interviews, onboarding, and orientation.
