Continuum Therapy Partners, LLC in Morris, MN is seeking a Physical Therapist Assistant to support clinician-led care focused on patient outcomes. The role emphasizes collaboration with therapists to ensure quality care and patient progress in a dynamic, multi-site organization.

You will implement individualized treatment plans under supervision, educate patients on home exercise programs, and help document patient progress during a daytime full-time schedule at West Wind Village.

Qualifications

• Associate Degree in Physical Therapist Assistant Program required.
• Current, unencumbered state Physical Therapist Assistant License.
• 1 year of previous experience as a Physical Therapist Assistant preferred.

Responsibilities

• Documents patient progress and reports changes in patient status to the supervising PT.
