Continuum Therapy Partners, LLC is seeking a Physical Therapist Assistant for a full-time daytime role at Whitewater Health Services in St. Charles, MN.

The PTA will implement individualized treatment plans under the supervision of a Physical Therapist, document patient progress, and educate patients on home exercise programs and assistive device use. Qualifications include an Associate Degree in Physical Therapist Assistant and an active state license, with 1 year of PTA experience preferred.

Qualifications

• Associate degree in PTA from an accredited program.
• Active state PTA license.
• 1 year PTA experience preferred.
• Performs treatment plans under PT supervision.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
• Communicates effectively with patients and care team to optimize outcomes.

Physical Therapist Assistant Requirements & Responsibilities

• Associate Degree in Physical Therapist Assistant Program required
• Current, unencumbered state Physical Therapist Assistant License
• 1 year of previous experience as a Physical Therapist Assistant preferred
