Job summary

Flagstar Rehab is seeking Physical Therapy Assistants (PTA) to work in Skilled Nursing Facilities. We connect compassionate PTAs with facilities that fit their lifestyle, offering a “choose the way you earn” model and support to establish their own corporations upon request.

Requirements: Current NYS PTA license and registration, valid BLS, and EMR competence. You will provide therapy in SNFs, collaborate with the PT supervisor, and monitor patient progress.

Qualifications

• Current NYS PTA license and registration.
• Valid BLS certification.
• EMR competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable)

Responsibilities

• Collaborate with the daily PT Supervisor and follow the established Plan of Care for all patients.
• Provide quality therapy services to individuals in SNF settings with varied diagnoses.
• Monitor and document patient progress.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Physical Therapy Assistant Requirements and Responsibilities:

• Current NYS PTA License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
