Job summary

Flagstar Rehab seeks a Physical Therapy Assistant (PTA) to join its skilled nursing facility teams across NYC and surrounding areas. You will collaborate with PT supervisors, deliver therapy to patients with TBI, CVA, stroke, Parkinson’s, MS, and other conditions, and assist in patient/family education to enhance function and cognition.

The role supports flexible scheduling (FT/PT/Per Diem) with a focus on high-quality care and consistent progress documentation using EMR systems.

Qualifications

• Licensed NYS PTA with current registration.
• EMR software proficiency.
• Authorized to work in the US in a nursing facility.

Responsibilities

• Follow Plan of Care with PT Supervisor.
• Provide therapy services to SNF patients with diverse conditions.
• Monitor and document patient progress.

Physical Therapy Assistant (PTA) in Skilled Nursing Facility

Now Staffing Physical Therapist Assistants for Rehab Department in state‑of‑the‑art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTAs work with a diverse and multi‑disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Hourly Pay Rate: $34-$36 Hourly (W‑2), $38-$40 Hourly (1099)

Physical Therapy Assistant Job Duties:

• Collaborate daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.

Physical Therapy Assistant Requirements:

• Current NYS PTA License and Registration
• EMR Competence
• Met all State and Federal requirements for US employment in a nursing facility
• Malpractice/Liability Insurance (if applicable)
