Flagstar Rehab is seeking Physical Therapy Assistants (PTAs) for Skilled Nursing Facilities in NYC and surrounding areas. You will work with a multidisciplinary team to provide treatments and educate patients and families to improve function.

We offer direct employment or independent contractor options, weekly pay, paid orientation, flexible schedules, and benefits. NYS PTA license required; EMR experience preferred; we can assist with setting up a corporation if needed.

• Meets all State and Federal requirements for US employment in a Nursing Facility

Physical Therapy Assistant (PTA) in Skilled Nursing Facility

Now Staffing Physical Therapist Assistants for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTAs work with a diverse and multi-disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Physical Therapy Assistant Job Duties:

• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.


• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.
