Flagstar Rehab is seeking a Physical Therapy Assistant (PTA) for our Skilled Nursing Facility program in NYC. You will work with a multidisciplinary team to deliver quality therapy and patient/family education to improve function and cognition.

Requirements include a current NYS PTA license, registration, EMR proficiency, and eligibility to work in the U.S. We offer options for direct employment or independent contractor roles, weekly pay, and comprehensive benefits.

Physical Therapy Assistant (PTA) in Skilled Nursing Facility

Now Staffing Physical Therapist Assistants for Rehab Department in state‑of‑the‑art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTAs work with a diverse and multi‑disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Hourly Pay Rate: $34-$36 Hourly (W‑2), $38-$40 Hourly (1099)

Physical Therapy Assistant Job Duties:

• Collaborate daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.

Physical Therapy Assistant Requirements:

• Current NYS PTA License and Registration
• EMR Competence
• Met all State and Federal requirements for US employment in a nursing facility
