Job summary

Flagstar Rehab is seeking Physical Therapy Assistants (PTA) to work in Skilled Nursing Facilities (SNF). This role supports geriatric patients and collaborates with PT Supervisors to implement treatment plans in SNF settings.

Offering flexible scheduling options including full-time, part-time, and per diem, with competitive hourly pay in NY. On-site positions with remote onboarding available for efficiency and speed to placement.

Qualifications

• Must hold a current NYS PTA license and registration.
• BLS certification is required.
• Experience with EMR systems is preferred or required.

Responsibilities

• Collaborate with PT Supervisor and follow Plan of Care for all patients.
• Provide quality therapy services to SNF residents with diverse conditions (e.g., TBI, CVA, Parkinson’s).
• Monitor, document, and report patient progress and outcomes.
• Adhere to state and federal employment requirements for nursing facilities.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Physical Therapy Assistant Requirements and Responsibilities:

• Current NYS PTA License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
