Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Pahokee, FL. This role supports the Physical Therapist in delivering resident-focused therapy services and implements treatment plans under supervision.

Full Time, Part Time, or PRN options are available. Requires an Associate's degree in physical therapy and an active PTA license, with ongoing continuing education to maintain licensure.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide PT services to residents under the supervision of a Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and treatment planning under PT direction.
• Contribute to short- and long-term goals and treatment plans.
• Perform individual and group therapy activities as outlined in the plan.
• Document resident progress and adjust care with guidance from the PT.
