Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility partner located in Pahokee, FL. The PRN role supports resident therapy services under a Physical Therapist's supervision.

Responsibilities include screenings, participating in evaluations, developing treatment plans, and delivering individual and group therapies focused on strength, mobility, gait, and functional independence.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying target goals and treatment plans.
• Conduct individual and group treatments as outlined in the plan including strength, mobility, gait, and ROM activities.
