Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Pahokee, FL. The PTA will work under the supervision of a Physical Therapist to provide individualized and group therapies, conduct screenings, and help implement treatment plans to improve resident mobility and function.

Responsibilities include documenting progress in NetHealth EMR, coordinating with the PT and care team, and supporting quality care standards.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide PT services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention or treatment.
• Assist in evaluation and in developing treatment plans and goals.
• Implement treatment plans including strength, mobility, balance, gait and ROM activities.
• Document care and progress in the NetHealth EMR per CMS expectations.
