Job summary

Evolve Therapy Services is hiring a compassionate Physical Therapy Assistant in Pahokee, FL for an in-house Skilled Nursing Facility. The role supports the Physical Therapist with patient care, screenings, evaluations, and treatment planning in accordance with CMS and state regulations.

Requirements include an Associate's degree in physical therapy, active PTA licensure in the practicing state, and ongoing continuing education to maintain licensure.

Qualifications

• Associate degree from an accredited program in physical therapy.
• Active PTA license in state(s) of practice; ongoing licensure maintenance required.

Responsibilities

• Provide physical therapy services to residents under supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment; assist in evaluations.
• Assist in treatment planning with target goals and schedules.
• Perform individual and group treatments including strength, mobility, gait, balance, ROM, and modalities.
• Assist in selecting techniques/media and adapt as needed.
• Monitor resident response and adjust as directed by PT.
• Document compliant and medically necessary notes in NetHealth EMR per CMS expectations.
