Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Pahokee, Florida. The successful candidate will work under the supervision of a Physical Therapist to provide targeted therapy services and support resident mobility and function.

Responsibilities include screenings, evaluations assistance, treatment planning, and delivering both individual and group therapy.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and treatment planning under the Physical Therapist.
• Deliver individual and group treatment including mobility, gait, ROM, and modalities.
• Document CMS-compliant, medically necessary therapy in NetHealth EMR.
• Train residents, families and staff in equipment use and care.

Formation

Associate's degree in physical therapy

Active Physical Therapy Assistant license

Continuing education to maintain licensure
