Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Le Center, MN. The role is PRN and involves assisting the Physical Therapist in delivering patient-focused therapy services on-site.

Responsibilities include screenings, developing treatment plans with goals, and conducting individual and group sessions. Requires an active PTA license in the state and an Accredited Associate's degree, with CMS documentation using NetHealth EMR.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and licensure maintenance to remain current.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and in developing short- and long-term treatment goals.
• Carry out treatment plans including strength, mobility, balance, gait, and ROM activities.
• Assist in documentation and CMS-compliant records in NetHealth EMR.
