Job summary

Evolve Therapy Services, LLC in Saint Paul, MN is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility on a PRN basis to support compassionate resident care. Under the supervision of a Physical Therapist you will conduct screenings, assist with treatment plans and goals, document in NetHealth EMR, and help educate residents and families.

Qualifications

• Associate's degree in Physical Therapy from an accredited program (must be current).
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in identifying a target list with short- and long-term goals and treatment plan.
• Conduct individual and group treatments as outlined in the plan.
• Assist in recording and documenting therapy activities in NetHealth EMR per CMS expectations.

Education
