Job summary

Evolve Therapy Services in Saint Paul, MN is seeking a qualified Physical Therapy Assistant for an in-house Skilled Nursing Facility on a PRN basis. You will support the Physical Therapist and help deliver planned therapy programs to residents.

Qualifications

• Requires ACCD in physical therapy and active PTA license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services under supervision of Physical Therapist.
• Conduct screenings to determine need for intervention.
• Assist in evaluation and development of treatment plans.
• Execute individual and group treatments per plan including strength, mobility, gait, balance, ROM.
• Educate residents, family, and staff on therapy techniques and equipment.
• Maintain CMS-compliant documentation in NetHealth EMR.
