Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Hopkins, MN. The role requires providing physical therapy services under the supervision of a Physical Therapist.

Full-time or PRN, with duties including screenings, treatment planning, and both individual and group sessions. Active PTA license and ongoing continuing education are required, with CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active PTA license in applicable states.
• Commitment to ongoing continuing education and professional development.
• Ability to complete CMS-compliant documentation in NetHealth EMR.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention.
• Assist in developing target goals and treatment plans.
• Deliver both individual and group therapy sessions per the plan.
• Adapt techniques and modalities based on resident performance.
• Maintain resident schedules in collaboration with the PT and team.
