Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility near Le Center, MN. This PRN position supports the supervising Physical Therapist and contributes to resident-centered care.

The role involves screenings, treatment planning, and both individual and group therapy. Successful candidates hold an associate degree in PT, an active PTA license, and commit to ongoing education; competitive pay and growth opportunities are offered.

Qualifications

• Associate degree in physical therapy from an accredited program is required.
• Active PTA license in the state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/tresreatment.
• Identify indications for evaluations and assist in evaluation under supervision of the Physical Therapist.
• Assist in establishing target lists, short- and long-term goals and treatment plans.
• Conduct individual and group treatment covering strength, mobility, balance, gait, ROM and modalities.
• Assist in selecting PT techniques/media and adapt as indicated by resident performance.

Education

Associate's degree in physical therapy from an accredited program

Active Physical Therapy Assistant license in state(s) of practice

Ongoing continuing education to maintain licensure and certification
