Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Saint Paul, MN. Full Time or PRN.

You will assist the Physical Therapist, conduct screenings, and participate in developing treatment plans to improve resident mobility and function. Responsibilities include documenting care in NetHealth EMR, coordinating with the PT and care team, and supporting resident and family education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the PT.
• Assist in identifying target goals and treatment plans with the PT.
• Conduct individual and group treatment as outlined in the treatment plan.
