Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Le Center, MN. The role is PRN, supporting therapy services under the supervision of a Physical Therapist.

Responsibilities include screenings, treatment plan contributions, and patient education, with documentation in NetHealth EMR. Requires an active PTA license and an accredited associate degree.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the Physical Therapist.
• Assist in establishing and modifying resident schedules with the Physical Therapist and team.
• Train residents, families and staff in use of therapy equipment and techniques.
• Document compliant and medically necessary therapy sessions in NetHealth EMR.

Education
