Job summary

Evolve-Therapy-Services seeks a Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role partners with licensed PTs to deliver patient-centered care and maintain CMS-compliant documentation in NetHealth EMR.

Responsibilities include screenings, treatment planning, individual and group sessions, gait and mobility work, and education for residents and families. A PTA license in the relevant state is required, along with ongoing professional development.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice; ongoing learning to maintain licensure.
• Commitment to continuing education and professional development to stay certified.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention.
• Assist in evaluations and treatment planning under the Physical Therapist's direction.
• Develop and implement target goals and treatment plans with the team.
• Deliver individual and group therapy including strength, mobility, balance, gait, and ROM activities.
• Select techniques/media and adapt activities based on resident progress.
• Monitor resident response and modify therapy as indicated by the Physical Therapist.
• Assist in scheduling in coordination with the Physical Therapist and team.
• Adhere to resident schedules and treatment regimens.
• Educate residents, families, and appropriate staff on therapy use and care.
• Participate in quality assurance activities and ensure CMS documentation in NetHealth EMR.

Education

Associate's degree in physical therapy

Active PTA license in state(s) of practice
