Job summary

Evolve Therapy Services, LLC in Pahokee, FL is seeking a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will work under a Physical Therapist, delivering services and helping implement treatment plans for residents.

The role requires completing screenings, assisting in evaluations, and coordinating plans with the PT and care team. Licensure must be current and ongoing continuing education is expected.

Qualifications

• Active Physical Therapy Assistant license in state(s) of practice.
• Associate degree in physical therapy from an accredited program.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and treatment planning under the Physical Therapist supervision.
• Execute treatment plans including strength, mobility, gait, balance, ROM activities and modalities.
• Monitor residents’ response to intervention and propose modifications with PT input.
• Coordinate resident schedules with the PT and care team.
