Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Florida. The role supports licensed Physical Therapists, conducts screenings, and helps implement treatment plans to improve resident mobility and function.

Responsibilities include individual and group therapy, documentation in NetHealth EMR, and collaboration with the care team to ensure quality resident care. Requires active PTA license and ongoing continuing education.

Qualifications

• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Associate's degree in physical therapy from an accredited program.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the Physical Therapist's direction.
• Assist in establishing treatment plans with short- and long-term goals.
• Conduct individual and group treatment including mobility, gait, ROM, and balance activities.
• Assist in selecting techniques/media and adapt as needed in consultation with the Physical Therapist.
• Monitor residents' response to intervention and adjust as indicated.
• Contribute to resident scheduling with the Physical Therapist and team.
• Assist with documentation and CMS-compliant records in NetHealth EMR.
