Job summary

Alternate Solutions Health Network, LLC is seeking a qualified Physical Therapist Assistant to provide in-home skilled services under the supervision of a Physical Therapist in the North Canton, Green, and Hartville areas. The role emphasizes patient education, documentation, and adherence to state guidelines.

Qualified candidates hold an active PTA license in Ohio, with at least one year of acute PT experience; home health experience is desirable.

Qualifications

• PTA license in the state of employment.
• Minimum of one year of PTA experience in an acute setting required.
• Home care experience is preferred.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate physical therapy judgments.
• Detail-oriented with disciplined work ethic.

Responsibilities

• Provides skilled PTA services in the patient home as per the plan of care.
• Records patient progress and reacts to changes.
• Performs PT procedures and preventive/therapeutic exercises.
• Educates patients and families on therapy techniques and safety.
• Completes clinical documentation and Medicare/agency requirements.
• Participates in case conferences and staff trainings.
